Geriatric Case Manager
Counseling Services
GAIL Program Geriatric Case Manager and Counselor Tom Patota, LCSW-C, provides free counseling sessions for up to six months for Greenbelt residents 60+ or disabled adults 50+ and their caregivers. The goals of the Counseling Program are to provide a safe and supportive environment for participants, to help clients address issues of concern, and maintain their independence.

Counseling services will be offered in the GAIL Program Office at 25 Crescent Road,First Floor, CARES office.  

In home counseling is available on an as needed basis with special consideration given to frail, high risk, homebound and/or socially isolated seniors. In-Home Counseling provides emotional support and telephone follow-ups to homebound and/or socially isolated seniors who would benefit from counseling and who otherwise would not have access to mental health services.

    • Anxiety
    • Grief and Loss
    • Isolation
    • Living withChronic Illness
    • Caregiver Stress

    Counseling services will be provided Monday through Friday with evening hours available by appointment. To schedule an appointment please contact 301-345- 6660 to complete a new client intake form.
