REQUESTS FOR SPECIAL ASSISTANCE

Requests for special assistance for Greenbelt Recreational programs can be made through the Therapeutic Recreation Coordinator. Please download the "Special Assistance Request Form." below.

WORD VERSION: The word version of this form may be filled out on your computer. You must "save as" , give it a new name and attach it as an email to Karen Haseley.

PDF VERSION: The pdf version can not be filled out electronically. You must print it out and mail it to: Karen Haseley, City of Greenbelt Recreation, 25 Crescent Road, Greenbelt, MD 20770.
